They require a Care Coordinator for a 12 weeks contract period to join their experienced Care Coordination team. They are located in the North West & Hills District - Sydney.

JOB DESCRIPTION

The Care Coordinator/ Care Manager, will support their customers with Home Care packages, visit their homes, complete assessments & reviews etc. You will need to have a strong case management and clinical background.

QUALIFICATIONS

Skills & experience required:

  • Clinical or Registered Nurse (RE) background
  • Currently registered with AHPRA or equivalent
  • Ability to engage with internal and external stakeholders
  • Case/Program Management experience within an aged/community services
  • Homecare package experience
  • Assessment and reviews experience
  • Excellent organization, time management and communication skills
