Location: Penn Yan, New York (Finger Lakes Region of New York State)

Education: Minimum: B.S. degree from an accredited physical therapy program.

License/ Certifications: License: Licensed, or license eligible to practice physical therapy in the State of New York. Certifications: BLS certification required through the RQI system during clinical orientation. 

Experience: Minimum: New graduate considered. Preferred: One to two years of experience. 

Other: Minimum: Must be able to travel to multiple locations on any given day as requested. Demonstrated ability to handle confidential information with discretion and ability to deal with the public in a professional and courteous manner. Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling. Excellent interpersonal, communication and organization skills. Computer literacy Preferred: Experience with Microsoft Office products and electronic medical record

Population Specific Requirements: The staff member must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients/customers served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient's/customer’s status and interpret the appropriate information needed to identify each patient's/customer’s requirements relative to his or her age-specific needs, and to provide the care needed by pediatric, adolescent, adult, and geriatric patient group. 

Summary: Under the direction of the Rehab leadership, prepares and carries out treatment plans that adhere to all policy and procedures of the facility and department. Maintains accurate and complete records, daily scheduling, and billing. Works collaboratively with PTA to delegate and oversee treatments. 

Essential Job Functions: 

Performs functionally based evaluations and establishes measurable time based goals. Selects appropriate treatment programs, monitors patient status, and communicates information appropriately. Establishes and conducts all patient care programs according to guidelines established in policy & procedure manual. Encourages direct patient and/or family involvement in program planning & goal setting. 

Completes all documentation (including contracted services) as stated in contracts or policy and procedure manual. Actively participates in providing the patient and other appropriate individuals with therapy information or training necessary to assure continuity in patient care. 

Communicates effectively with patients, physicians, visitors, volunteers, students, and other health care personnel in a manner that conveys professionalism. Attends and actively participates in patient care, departmental and organization meetings in a manner that demonstrates a cooperative and positive attitude. Fosters collaboration with Rehab department to achieve patient goals/objectives. Actively supports and participates in the customer oriented team approach. 

Demonstrates an ability and willingness to individualize the approach to a patient regardless of patient's personality, disability, or psycho-social needs (i.e., patient fears, concerns, or changes in life styles). Modifies the treatment program to best suit the needs of the patient.
