Summary: The Hospice Evaluator is a community health nurse (CHN), registered nurse or Social Worker whose responsibilities and functions are concerned with the care of the terminally ill patient admitted to Hospice. 

Essential Responsibilities/Accountabilities: 

The functions of the Hospice Evaluator include, but are not necessarily limited to, the following: 

• Assess patient and family needs and determines their eligibility for care in the Hospice program. 
• Establish the initial plan of care with the Interdisciplinary Group (IDG). 
• Participate in Interdisciplinary group meetings as assigned. 
• Consistently demonstrates high standards of integrity by supporting the Companies’ mission and values and adhering to the corporate code of conduct. 
• Maintain high regard for member privacy in accordance with the corporate privacy policies and procedures. 
• Regular and reliable attendance is required. 
• Performs other functions as assigned by management. 


Minimum Qualifications: 

• Graduate of an accredited school of registered nursing and a valid NYS RN license required, or Bachelors degree in nursing or one year of home health care nursing experience. CRNH preferred. Or Bachelors degree in Social Work or related field. 
• A minimum of one year of related experience and knowledge of Hospice regulations required. 
• Must have an interest in and sensitivity for the care of terminally ill patients and their family. 
• Ability to travel required. 
• Must demonstrate solid interpersonal, organizational and time management skills. 
• Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents, if necessary. 
• With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary. 

Physical Requirements 

Ability to travel as required for a professional home visitor.
