JOB DESCRIPTION

RN- Discharge Planner - FULL TIME

Direct HIRE

Monday - Friday. 8hr Days

Quick Start

On St. Lawrence River

Min 3 yrs experience 

EDUCATION, EXPERIENCE, CERTIFICATION, LICENSURE: 

• Completion of all RN credentialing procedures and requirements 
• Experience in discharge planning preferred 
• Experience in a Health Care setting with knowledge of community resources preferred 
• Good communication and writing skills
• Must have current active license or eligible in good standing

JOB DESCRIPTION: Works with physicians and all other members of the Interdisciplinary health care team to assist those patients and their families who present with difficulties in coping with personal, social, and discharge planning needs and to coordinate a safe, effective discharge plan for patients/families identified as high risk in preparation for post-hospital needs. 

Coordinates the admission process for Swing Bed patients. Serves in the role of patient advocate relating to the complaint procedure. Provides an ongoing program of activities to meet, in accordance with the comprehensive assessment, the interests and physical, mental, and psychological well-being of each patient.
